** BEFORE YOU REGISTER FOR CVMS **
 
Students currently attending Solana Pacific, Carmel Del Mar, Sage Canyon, Ocean Air and Torrey Hills Elementary Schools are NOT automatically scheduled to attend CVMSYour HOME ADDRESS will determine which middle school your student will attend.  Click the Boundary Locator link below to enter your home address and discover which middle school your student will attend.  If you do not live within the CVMS boundaries, but still want your student to attend, you MUST have an APPROVED Intradistrict Transfer on file at the District Office.  Click the link below for more information.
 
 
 
Enrollment Forms for 2017 - 2018:
Please follow the checklist so all forms will be submitted correctly and remember to include:
  • A copy of the student’s birth certificate
  • Copies of 2 proofs of residency (one must be a current electric bill)
  • A copy of a complete immunization record. All dates must be signed or stamped by a doctor or clinic. We will not call your child's prior school for these records. 
  • Refer to the Required Immunization Information New Law- The California School Immunization Law Requires that children be up-to-date on their immunizations (shots) to attend school.                                                                                                                                                                                          
 Your enrollment will not be processed unless all required items are included.

PLEASE NOTE:
Forms are digitally fillable except for the Carmel Valley Enrollment checklist.
 
All forms must be printed single sided for sorting purposes.
 
Incoming students from outside of our feeder school area need to provide a copy of your child's most recent report card and standardized test scores
